Age 8+ and identified by an agency or professional as having one of the following conditions: intellectual disabilities, cognitive delays as measured by formal assessment, or significant learning or vocational problems due to cognitive delay that require or have required specially-designed instruction.

Application process
Applicants should call or mail the office. They will be provided with an registration packet and guidance for contacting a team within their region. On receipt of the completed SOKS Medical Release signed by a qualified physician SOKS will approve individual for participation. Individuals are assigned teams depending upon location.

Program of sports training, athletic competition, health promotion, and health education for children and adults with developmental disabilities ages 8 and above. Individuals can participate in up to 20 sports depending upon ability. There are 107 teams in 5 regions within the state. In addition, there are extensive health and wellness and leadership programs as well as Unified Sports opportunities (disabled and non-disabled athletes competing together).
