Eligibility
Anyone who is 60 years of age and over
- Or those asking on behalf of someone 60 years of age or over. Females served, ages 60 to 150. Males served, ages 60 to 150.
Required documents
Must complete an agency registration form (NAPIS) and assessment forms.
Social Security Card, Proof of Residence, Medicare Card might be required to apply for some benefits.

Long Term Care Ombudsman, Information and Assistance, Care Coordination, Senior Centers with Meal Programs, Home-Delivered Meals, Respite Care, Transportation and Assistance with Public Benefits including Medicare Outreach, Education and Enrollment; ACA Marketplace Health Insurance assistance. Services include congregate meals/nutrition sites, senior ride programs, elder law, certified application counselor programs, Medicare information/counseling, adult day program centers, case/care management, adult protective services, adult in home respite care, area agencies on aging and specialized information and referral.
