Case Management: Age 18 years and older chronically mentally ill
MHJJ (Mental Health Juvenile Justice):
Youth ages 12-18 years with mental health issues
Youth ages 12-18 years who are in the detention center
Accepts youth ages 12-18 years who have the state medical card or no insurance
Required documents
Case Management: Proof of Income, Insurance Card
