Age 18 years and older
  • Individuals age 60 years and older with a diagnosis of Traumatic Brain Injury (TBI) or HIV/AIDS
  • Illinois resident
  • US Citizen
  • Living in an institutionalized setting for a minimum of 90 days
  • Must be Medicaid eligible or receiving benefits
  • Physician approval for a safe transition
