Eligibility

Must be a child under 5, or a pregnant or postpartum/breastfeeding person & have a nutritional need (WIC staff can help you determine this), & have a family income less than WIC guidelines (185% of proverty level). If you currently receive TANF, Food stamps or Medicaid/MC+, you are automatically income eligible.

Required documents

Photo ID for patient, 1 proof of address document (from within the last 30 days) and 2 proof of income documents (pay stubs) or income tax return if discounted fee, proof of income for each household member 18 & older as will have to verify household size
