Provide Care Coordination to older persons and their family caregivers, including assessment, care plan development, monitoring and assistance in accessing public and private services.
Provide assistance with Medicare Part D and Missouri Rx application and enrollment process. Can also advocate for clients experiencing any problems with those programs. Service includes case/care management.

Go west on Highway 28 to the first 4-way stop light. Turn right onto First St. and go approximately three blocks. The Center is on the right. Use the large parking lot beside the brick building.
